Online Access

To access your records online, you can:

1. Download the NHS App from the App Store on your phone. You will need to know your NHS number and this can be provided to you upon request by calling or e-mail our reception staff.
2. To access your GP-held records, please come to reception and ask for your personal ID and Password.
3. All requests for printed records should be sent to our e-mail: M85128.waljimt@nhs.net. Please include your name and date of birth in the request.
